-서론-
신생아학의 발달로 미숙아를 포함한 신생아 중환자의 생존율이 향상되고 있으나 우리나라
의 신생아 중환자 관리에는 아직 많은 문제점이 있어 이에 대해 살펴보고자 한다.
신생아라 함은 생후 28일 이내의 유아를 말하며, 재태 연령 37주미만의 미숙아나 출생 체
중 2,500그램 이하의 저출생체중아를 포함하여, 외과적 수술을 요하는 선천성 기형을 가진
신생아들이나 패혈증 등의 중한 상병을 가진 신생아들이 신생아 집중치료실 혹은 신생아 중
환자실(이하 신생아 집중치료실로 표기)의 입원 치료대상이 된다.
이들 신생아 중환자들에 대한 관리가 제대로 되기 위해서는 최신 의료 기술의 도입과 더
불어 적절한 인력, 시설 및 장비, 진료 전달 체계 등이 갖추어져야 한다. 우리나라의 신생아
중환자 관리에 대한 이러한 여건은 어떠하며 문제점은 무엇인지를 알아보고 그 대책을 생각
해 보고자 한다.

Regionalization of the perinatal care had already been established in the developed countries. But the perinatal care is not yet organized here in Korea.
Three main factors are related to the lack of the organized regional perinatal care system. Fist, there are insufficient numbers of the neonatal intensive care beds in the tertiary hospitals, and physical facilities including equipments. Only twenty-seven percent of the estimated bed requirements for intensive care was available and the numbers of the equipments such as monitors, ventilators, infusion pumps, incubators and infant care systems were also only twenty to thirty percent of the requirement. The next factor is personnel shortages, of both doctors and nurses. The number of patients a nurse has to take care of is about five times the number recommended by the American Academy of Pediatrics and American College of Obstetricians and Gynecologists. The last factor is inadequate medical fee for the neonatal intensive care which were regulated by the government. Fee for the incubator care is only 4,500 won(about 5 US dollars) per day. Moreover, only 18,330 won(about 21 US dollars) can be charged for the ventilator care per day. It is insufficient not only for the renewal of the equipments every five years, but also for supporting the cost of the maintenance of the equipments.
By recruitment of medical personnels and physical facilities including equipments through improvement of the medical insurance policy, a regionalized neonatal intensive care system can be established in Korea.
